OUR SERVICES - SHORT-TERM REHABILITATIVE SERVICES
D'Youville Senior Care's specially designed rehabilitation program provides short-term services to patients who need rehabilitation and nursing care while transitioning from the hospital to home or other level of care.
Our nursing team has designed a program for patients recovering from a variety of illnesses and injuries including:
- Stroke (CVA)
- Hip fractures and replacement
- Knee replacement
- Shoulder surgery
- Cardiac and Pulmonary conditions
- Pelvic fractures and other orthopedic conditions
An interdisciplinary team of nurses, dietitians, physical, occupational and recreational therapists, along with speech and language pathologist and social services personnel work under the direction of a physician to provide intensive services for the short-term patients.
Our patient's progress is monitored closely by our interdisciplinary team who coordinates the discharge and assures all community support services are in place.
